Coral Gables & Greater Miami — In your home
Concierge Physical Therapist — Neurological & Orthopedic Care
Expert, one-on-one physical therapy brought into your home, with the time, attention, and 39 years of experience that real healing requires.
The Approach
For nearly four decades, I practiced inside the walls of private clinics, inside schedules, insurance approvals, crowded waiting rooms, and the relentless pace of productivity metrics. I witnessed extraordinary healing. I also watched care become compressed and healing become transactional. Patients became appointments.
So I changed how I practice, not what I believe. In-home physical therapy is my return to the essence of this work: human connection, individualized care, and the conviction that people heal best when they feel seen, heard, and truly cared for. The home is one of the most powerful healing environments there is. It reveals how you actually move, live, and want to live, so every exercise serves a real purpose, not an artificial one.
Not maximizing appointments. Maximizing outcomes.
Not treating diagnoses. Treating people.
Not therapy in a clinic. Therapy in your life.
About Carla
I'm Carla Ruhl. Born on the Eastern Shore of Maryland and raised in South Florida, I trained as a physical therapist at Florida International University and have practiced here ever since. I began my career at Jackson Memorial Hospital, then spent 32 years at Miami PT Associates, working with a wide range of patients, including those with spinal cord injuries, Parkinson's and other neurological conditions, post-spine surgery recovery, osteoporosis, and joint replacements.
That range taught me something no protocol can: how to read the whole person in front of me, not just the diagnosis on the chart. I hold certifications in LSVT BIG for Parkinson's, Kinesiotaping, and the Cionic Neural Sleeve, but the most important thing I bring into your home isn't a technique. It's presence.
When I'm not working, you'll find me traveling, cooking, exploring art exhibitions, or spending time with the people I love. I believe a full life is the whole reason to stay mobile, and helping you keep yours is why I do this.
What I believe
This isn't a retirement from physical therapy. It's a return to its essence.
What I treat
My practice is built on two areas of deep specialization, neurological and orthopedic care, and on decades of knowing where expert, individualized attention makes the greatest difference.
Rebuild core strength, stability, and confident movement after back and neck issues, through controlled, low-impact exercise tailored to your body and your goals.
Move through your home and your life with steadiness and confidence, and reduce the risk of falls that can change everything.
Return to the movements that matter: stairs, standing, getting in and out of a chair. With a plan built around your actual home, not a generic protocol.
Specialized, research-based programs, including LSVT BIG, to help maintain mobility, strength, and independence as life changes.
Hospital-level experience supporting strength, function, and renewed independence, delivered with patience and continuity, in the place you live.
How it works
A real conversation about your history, your body, and the life you want to get back to.
A personalized program built around your home, your goals, and the activities that matter to you.
Unhurried sessions, steady progress, and a plan that evolves with you over time.
For referring physicians
When you refer a patient to me, you refer them to one experienced clinician, not a rotating staff. I bring 39 years of neurological and orthopedic practice, including 32 years in outpatient physical therapy, along with hospital training at Jackson Memorial and certifications in LSVT BIG and the Cionic Neural Sleeve. Alongside that clinical work, I've spent the past 20 years caring for patients in their homes, so your referrals receive attentive, in-home expertise and clear, consistent communication back to you.
Your patients get presence and continuity. You get a partner you can trust.

Older adults who intend to stay independent and in their own homes; athletes and active people recovering from injury or surgery; and anyone navigating a neurological condition who wants an experienced, steady hand. If you're unsure whether it's a fit, call me and we'll talk it through.
Sessions are unhurried and built around your needs. We'll set a rhythm together based on your goals and progress, not a fixed quota.

As a concierge practice, my focus is unhurried, one-on-one care. I'll walk you through fees and payment options directly and transparently, so you can decide what's right for you.
In their words
From the moment I was injured in 1992, Carla has been a steadfast presence — guiding me through recovery and maintaining my health for over 33 years. She listens, understands, and adapts, pushing me to reach goals I once thought were out of reach.
Carla has worked with my 88-year-old mother for seven years and become far more than a physical therapist — a trusted advisor and a steady presence. She treats the whole person, attuned to my mother's dignity as much as her body, and has built a circle of care around our entire family.
I'm HERE FOR YOU
If you or someone you love is ready for physical therapy that's personal, unhurried, and genuinely expert — I'd love to talk.
After 39 years, I've learned that the most important part of physical therapy isn't the equipment, the clinic, or the protocol. It's presence. And that's exactly what I bring into every home I enter.
